Every child has their own path.
We help them walk it.

Pathways to Horizons is patient, one-to-one support for children with disability, in your home and out in the community. We build everyday skills, confidence, and independence, one small step at a time.

Who we walk beside

Made for children, and the families right beside them.

Some children do their best learning with one calm, familiar person who really knows them. That is who we are for. We work with children with disability on the everyday skills that open up their world, from getting ready in the morning to joining in, speaking up, and doing more on their own.

You know your child better than anyone. We follow your lead, keep you in the loop, and celebrate the wins with you.

What we do

Support built around your child, not a schedule.

Every plan starts with your child's own goals. Here is the kind of support we bring.

Everyday life skills

Routines, self-care, communication, and the small independence wins that build real confidence.

One-to-one attention

The same familiar person each time, so your child feels safe and progress can actually build.

At home and out and about

Support where life happens, from your living room to the park, the shops, and the community.

Working toward real goals

We set clear, meaningful goals with you and keep gently working toward them, week by week.

The path we walk together

Getting started is simple.

No pressure, no jargon. Three easy steps to begin.

1

Say hello

A relaxed chat about your child, your family, and what you are hoping to see. We listen first.

2

Get to know your child

We spend real time understanding how they learn, play, and feel safe, so support fits from day one.

3

Walk it together

A simple plan built around their goals, then steady, patient support that grows right alongside them.

Meet Lynne
"I started Pathways to Horizons to give families the patient, personal support I always wished I could offer."

I have spent years supporting children with disability, and the part I love most has never changed: being there for the moment something finally clicks. I keep things small on purpose, so every child gets someone who truly knows them, and every family gets a real person who answers the phone. When you work with us, you work with me.

Our promise

New, small, and completely on your side.

We are just getting started, and that is exactly why families feel the difference. There is room here for your child to be a priority, not a number.

One consistent person

Your child sees the same familiar face, so trust and progress build the way they should.

Honest, plain advice

We will always tell you what we think is right for your child, even when it is not more hours with us.

We start where they are

No fixed programme to squeeze into. We meet your child exactly where they are and move at their pace.

Because we are new, we do not have a wall of reviews yet. What we can promise is care, patience, and full attention from day one.

Where we work

Close to home in Melbourne's east.

We are based in Ringwood North and support families across the eastern suburbs. Happy to travel a little further when it is the right fit, and growing across Victoria as our small team grows.
